Contact Information Update Form
Use this form to update your address, email, or phone number.
Document Request Form
Use this form to request copies of documents that you have submitted to BRHP in the past.
Employment Self-Certification
Use this form to certify your employment.
Housing Search Assistance Request
Interim Change Package
Use this form to report any changes in income or household composition. Changes must be reported to BRHP within 30 days.
Mediation Request Form
Use this form to request a 30 minute Zoom appointment with a BRHP Counselor, tenant, and landlord.
Optional Contact Form
Use this form to provide an additional contact person in case issues arise and you are unable to be reached.
Request to Move Form
Use this form to request to move and begin the move process.
Unaddressed Maintenance Issue Report Form
Use this form is to report issues that you have previously reported to your landlord that have not been addressed.
Voucher Extension Request
Use this form if your voucher is approaching expiration and you need an extension.
Withdrawal Form
Use this form if you would like to voluntarily withdraw from the BRHP program.

211 Maryland

2-1-1 Maryland is a partnership of 4 agencies working together to provide simple and easy access to health and human services information. The 2-1-1 database has information on more than 7000 agencies and programs across the state. Each week 2-1-1 handles thousands of calls from people in need, providing referrals to services and helping people problem-solve when services they need are not available.

BRHP Search Tool

The BRHP Search Tool allows any applicant, resident or property partner to quickly determine if a home is in an Opportunity area, and provides an estimated range of rents based on the payment standard. The tool provides valuable neighborhood context, including transit lines, grocery stores, child-care centers and school information. The tool displays the zoned schools for that particular home, as well as the GreatSchools score and a link to the GreatSchools page for each school.

HUD Housing Assistance Payment (HAP) Contract

The HAP contract is a written agreement between the PHA (BRHP) and the owner of a unit occupied by a housing choice voucher program participant. The HAP contract must be in the form prescribed by HUD. Under the HAP contract, the PHA agrees to make housing assistance payments to the owner on behalf of a specific family leasing a specific unit. The PHA uses its payment standard schedule to calculate the monthly HAP payment to the owner.

The Office of the Attorney General’s Consumer Protection Division has created this publication to help renters and residential property owners understand their respective rights and obligations, as well as the remedies that are available under Maryland law. It covers a broad range of issues, such as applications, leases, security deposits, rent escrow, lead-based paint hazards, eviction, and where to seek help if problems arise.

Violence Against Women Act (VAWA)

The Violence against Women Act of 2013 (VAWA) provides special protections for victims of domestic violence, dating violence, sexual assault and stalking who are applying for or receiving assistance under a Housing Choice Voucher program.  Protections are not limited to women but cover victims regardless of sex, gender identity, or sexual orientation.  If state or local laws provide greater protection for such victims, those laws apply along with VAWA. Under VAWA, BRHP allows tenants who are victims of domestic violence, dating violence, sexual assault, or stalking to request an emergency transfer from the tenant’s current unit to another unit.

Walk Through Checklist

Use this form so both you and the landlord have a record of your unit’s condition before you move in and when you move out.  Keep a copy for yourself and one for the landlord.
