Document Request Form
Use this form to request copies of documents that you have submitted to BRHP in the past.
What types of documents can a Head of Household request?
Using this form the Head of Household can request a copy of a family member’s birth certificate, an employment verification, a family member’s ID, their lease, a family member’s social security card, their SSI/SSA letter, their voucher, and/or their C & L letter (the letter that states the tenant’s portion of rent).
How long will it take to process my request?
It can take up to 3 business days to process your document request.
How will I receive my document(s) once the request is processed?
You can receive the document(s) by mail or by coming in person at our office. You must choose which option you prefer in the document request form.
If I decide to pick up my documents at the office, what is the procedure?
If you choose pick up on the document request form, the item will be available at BRHP’s front desk in 3 business days for pick up by the Head of Household. The Head of Household must show photo ID to pick up item(s) at the front desk. The Head of Household must sign the Document Request Form when they pick up their item(s) at the front desk.
If I decide to receive my document(s) by mail, what is the mailing procedure?
If you choose to receive the document(s) by mail they will be mailed in 3 business days to the address that we have on file for you.
